Nurse Practitioner Clinic Hours:

• Mondays: 8:30AM - 1PM
• Tuesdays: 8:30AM - 5PM
• Wednesdays: Closed
• Thursdays: 8:30AM - 5PM
• Fridays: 8:30AM - 1PM

Please contact Health Services to schedule an appointment if you are unable to attend during regular clinic hours. Appointments are required for all physical exams. If you need to cancel, please provide at least two hours’ notice.

Physician Hours: Designated Friday afternoons. Doctor appointments cannot be made online. Email Health Services for an appointment.

• Residential students must submit ALL five health forms.
• Non-residential, full-time Day Program undergraduate students must submit the Physical Exam Form and Health History Form. They are not required to submit the Immunizations Form, Meningitis Form, or Tuberculosis Test Form unless required by their program (for example, Nursing).
• Online and graduate students do not need to submit any health forms.
• ALL full-time Day Program undergraduate students must sign and date the Health History Form section acknowledging receipt and review of the Privacy Practices.

To upload your health forms:

1. Log in to WesPortal.
2. Click the Student tab on the top menu bar.
3. Click Health Records & Forms on the left menu bar.
4. Click Upload Health Forms . Allow approximately one minute for the forms to attach before clicking submit.

A student health insurance plan fee of $2406 is automatically posted to all eligible registered, full-time, Day Program undergraduate students' accounts, payable with tuition and other fees.

All full-time traditional students, full-time non-traditional students who attend day classes, and all students living on campus are required to have health insurance that meets College waiver requirements.

Students must complete the waiver process before the designated deadline. Students who do not complete the waiver by the deadline will be enrolled in the student health insurance plan and charged the annual fee.

• International students must pay this fee and will be automatically enrolled in the student health insurance.
• Domestic students with acceptable ACA insurance may waive the student insurance. Complete the insurance waiver .
• Students who need to enroll in the student insurance plan may complete the enrollment form here .
