Medical Release form
  • Residential students must submit ALL five health forms.
  • Non-residential, full time Day Program undergraduate students must submit the Physical Exam Form and Health History Form, and are not required to submit the Immunizations Form, Meningitis Form, or Tuberculosis Test Form (except if required by their program; e.g. Nursing).
  • Online and graduate students do not need to submit any health forms.
  • ALL full-time Day Program undergraduate students must sign and date the Health History Form section to acknowledge receipt and review of the following Privacy Practices. If you have any questions regarding the privacy practices, please contact Student Affairs. Thank you.

To upload your health forms:
  1. Log in to WesPortal
  2. Click the “Student” tab on the top menu bar
  3. Click “Health Records & Forms” on the left menu bar
  4. Click "Upload Health forms" (allow 1 minute for the forms to attach before clicking submit).

A student health insurance plan fee of $2406 is automatically posted to ALL eligible (registered, full-time, day program, undergraduate) students' accounts, payable with tuition and other fees. Please read on for important details below.
 
All full-time traditional students, full-time non-traditional students who attend day classes, and all students living on campus are required to have health insurance that meets our college waiver requirements. Students must complete the waiver process on the CHP website before the designated deadline. Students who do not complete the waiver by the deadline will be enrolled in CHP insurance and charged the annual fee on their college account. Students who want to enroll in the CHP insurance must go to the CHP website before the deadline to enroll.

"As of August 1, all students who have not completed the online insurance form waiver and received an approval via email will see a preliminary charge for the student health insurance plan. After August 1, the waiver will remain open through August 14; the insurance charge will be credited within two business days after the student receives an approval email following the submitted waiver. Note: The waiver option and separate insurance charge are not for international students, who will have the insurance cost included in their international student fee. The annual group insurance plan charge will be $2406 for the annual term August 1, 2026 - July 31, 2027 (pro-rated for students ending in December or entering in January) posted to eligible (registered full-time, day program, undergraduate) students' accounts without an approved waiver as of August 1, payable with tuition and other fees. Please read on for important details below.

  • International students must pay this fee and will be automatically enrolled in the student health insurance.
  • Domestic students who have acceptable ACA insurance may "waive" the student insurance to receive a credit of the insurance fee or must "opt in" the student insurance if they do not have active acceptable insurance as of August 1 (January 1 if entering in spring at pro-rated cost). Between June 1 - August 1, please click here to get to the online form to waive (click waive).
  • If you do not have insurance, please enter the enrollment form here (click "Opt-In") by the policy start date of August 1 (or January 1 if entering in spring). The insurance fee will be applied August 10 without an approved waiver; this applies each annual term. Note: The insurance cannot be cancelled should you want to switch to another policy during the coverage period, which is August 1, 2026 - July 31, 2027.

Nurse Practitioner Clinic Hours:

  • Mondays: 9:00 - 11:30 AM
  • Tuesdays, Wednesdays, Thursdays, Fridays: 9:00-10:30 AM & 1:30-3:00 PM

Make an Appointment

Please contact Health Services for an appointment if you cannot come during clinic hours. Appointments are required for physical exams. Please cancel your appointment at least 2 hours in advance.

Physician Hours: Designated Friday afternoons. Doctor appointments cannot be made online. Email Health Services for an appointment.

 

Health Services is located in the Olive Swann Porter building (OSP). Health Services is staffed by a family nurse practitioner who provides medical care for common illnesses and minor injuries. A physician is available on campus by appointment. Students with chronic illnesses may be referred to a local physician for management. Health Services provides personal health education and educational programs on health-related topics. All information pertaining to services provided is confidential.

Students may be referred off campus for diagnostic tests or treatment beyond the scope of Health Service’s staff. Transportation and charges for off-campus services are the student's responsibility. Students should verify that the facility or provider is covered under their insurance plan. In an emergency, campus police will contact 911 or transport a student to the nearest emergency facility.

Navicent Health Urgent Care Northwest on Zebulon Rd. and CVS Pharmacy Minute Clinic on Zebulon Rd. are available for urgent medical care when Health Services is closed. For life-threatening emergencies, Coliseum Northside Hospital on Charter Boulevard and Navicent Health on Pine Street provide emergency treatment.

    A student health insurance plan fee of $2,444 for students entering in August (or $1,420 for students entering in January) is automatically posted to ALL eligible (registered full time, day program, undergraduate) students' accounts, payable with tuition and other fees. Please read on for important details below.
